Medical Clearance for App?

Hello everybody

I am thinking about applying to the program for the 2010 to 2011 school year. I know one of the things needed is a MEDICAL CLEARANCE.

I actually have my regular checkup with my doctor this week, and I was wondering if there was a specific form I needed to give to her. Or do I just ask the doctor for a medical clearance form and whatever form they give is fine?

Hi serendipity,

I'm gathering materials so I can apply for the 2010-2011 year as well. You don't need to submit a medical clearance with the application.  If you are selected, then you will need to submit the medical clearance and police clearance (both certified by the Apostille of the Hague) with your visa application. The medical clearance will need to be dated within 3 months of your visa application, so you should wait to get it done...

4. I have a question regarding the medical certificate. Is there a specific form?
A simple statement by your physician is enough. In most cases we receive statements like this (this is only an example; it is the doctor who has to decide about the text):
I, Dr _____________, as the primary care physician for ____________________ her/him, do hereby declare that ____________________ (your name here) is free of contagious disease, drug addiction, and mental illness. Ms./Mr.______________ (you family name here) is a very healthy individual in all senses, she/he has no pre-existing medical conditions, and she/he capable of travelling abroad.
Signature (Dr. ______________) Date _____________.

I wanted to bump this topic up and see if anybody else knows if you need to submit a police record and medical clearance with your application for the program, or if you ONLY need it for the visa application process.  On the faqs the previous poster listed, it appears that the medical clearance is definitely required for the program application, and I can´t tell if the police report is or not (for a US citizen). 

If anyone knows the answer to this question, I´d appreciate it!  Thanks!
